Pharmacists at the Alexian Anticoagulation Clinic

An Alexian pharmacist certified in anticoagulation therapy is available each weekday, by appointment, to monitor your warfarin treatment. The pharmacist and other clinic staff members will work with you to accomplish the following goals:

• Manage and monitor your anticoagulation dosing and treatment
• Deliver ongoing education and counseling
• Screen for drug-drug and drug-food interactions
• Communicate with your doctor, other healthcare professionals and family members as needed
