On-site Health Screening
Screenings are performed at a convenient site for your organization's members. Because Alexian Brothers Hospital Network is a local healthcare provider, we're flexible in terms of assigned times and locations. We measure:
- Cholesterol level through finger stick or blood draw
- Blood sugar level through finger stick or blood draw
- Body composition analysis
- Blood pressure
- Height
- Weight
Web-based health questionnaire
A 15-minute confidential health risk appraisal is available to participants via our website. It assesses behavioral risk factors including:
- Diet
- Exercise
- Sleep
- Smoking status
- Family History
- Past health issues
An extensive health profile is constructed for each participant. The individual can then understand his or her unique risks. In addition, PIP reports are actionable and positively impact your organization's health. The company's aggregate report will be delivered in person by Partners in Prevention's medical director. We will discuss the findings in detail and recommend actions to improve your organization's health. These may include physician consultation, wellness planning, health benefits design and/or health coaching. A sample of one of our reports can be viewed here.
Participants can go to our PIP web portal to access their personal health data. They can then find detailed health information pertinent to their own profiles. Privacy is assured throughout this process.
