Hospice is a unique health related service as the focus of care is not just on our patients and their specific needs, but also on the families of the patients. The focus of our services is to provide physical, emotional and spiritual support to our patients and their family members so that they are able to live their lives as fully as possible, to the end of their days, in comfort and dignity, surrounded by their loved ones. The Hospice Foundation of America offers a comprehensive explanation of hospice services.
The Alexian Brothers Hospice team is comprised of the referring physician, our medical director, nurses, hospice aides, social workers, chaplains, trained hospice volunteers, as well as the patient and the family. It is our goal to listen to the needs of the patient and family, and work together with them to provide the appropriate services.
Hospice services are offered at the patient's home...wherever that is...a private home, an assisted living facility, a nursing home, or in some specific situations, in the hospital.
In addition to the above, our hospice team also arranges for needed medical equipment, medications, and referrals to community resources.
Hospice personnel are available 24 hours a day, seven days a week, 365 days a year to respond to emergency situations that may arise with our patients.
