Telemonitoring

About Us

Many caregivers are challenged by the task of monitoring their homebound loved one's health on a regular basis. This is especially true for conditions that fluctuate daily or even hourly, such as blood pressure, heart and breathing rate and blood glucose.

Fortunately, new technology allows this type of monitoring without your loved one even leaving home. It is called telemonitoring or telecare and allows your loved one to have their vital health markers checked each day or several times each day, depending on the need. Telemonitoring helps your loved one better understand their condition and manage their health while also giving healthcare providers important information about your loved one's condition. This technology has also been shown to improve health outcomes for those living with chronic conditions. Telemonitoring has allowed many people to remain in their homes and live productive lives by addressing their health needs early, before hospitalization is needed.

Our Technology

Here's how the technology works: A monitor, along with the necessary equipment for checking vital signs, is placed inside the home. The monitor prompts your loved one to take their vital signs at specific time(s) each day. In just a few minutes, it collects heart rate, blood pressure, oxygen saturation, body weight and temperature. Additional data, such as glucose monitoring, lung function, blood-clotting times and heart rhythms can also be obtained.

The system then asks a series of subjective questions - through a pre-recorded voice - to further assess the patient for signs and symptoms of deterioration. The patient responds to each question by pressing a "yes" or "no" button on the monitor. Once completed, the data is transmitted via digital wireless technology to a secure web site for Alexian Home Health Care to review. If vital signs are outside the pre-established ranges set by the physician or nurse, the registered nurse contacts the patient and/or their physician to provide any necessary follow-up care. That care may be an additional nursing visit as directed by the physician, medication changes, or the recommendation that the client makes an immediate visit to their physician.
