About the Alexian Brothers Home Health Care Program

Alexian Brothers Home Health Care Program provides a continuum of care with a holistic focus using a multidisciplinary team approach. The ultimate goal is for your loved one to achieve as much independence and quality of life as is possible.

Our multidisciplinary home health team consists of registered nurses, physical, occupational and speech therapists, social workers, home health aides. We also have individuals equipped in pastoral care that are available to address spiritual needs.

Home care is generally provided for people who have:

• Diabetes
• Cardiac conditions
• Wound care
• Orthopedic issues such as post-surgery rehabilitation
• A need for intravenous antibiotic therapy
• A need for medication management and administration
• Disease management needs
• Early high-risk pregnancy problems
• A need for bridge care leading to hospice

The Alexian Brothers Home Health Care Program is certified by Medicare and the Joint Commission and licensed by the Illinois Department of Public Health.
