Advance Directives at Alexian Brothers Behavioral Health Hospital
Advance Directives are documents in which you state your wishes for your medical care in the event that you are unable to speak for yourself. In a Durable Power of Attorney for HealthCare you appoint an agent who is empowered to speak for you on medical matters at any time you are unable to speak for yourself. Patients or visitors may ask for a Chaplain to assist them in formulating a Living Will, Health Care Power of Attorney, or both documents. These documents are available in a booklet titled "Planning Beyond Tomorrow" and are provided by the Medical Center for your use.
A Chaplain is always available to discuss these documents with you. Neither an attorney nor a notary public is required to complete these forms, though you may use either professional to assist you.
