Schedule an Appointment at an Outpatient Group Practice

To schedule your first appointment, please call our Intake Department at 847-755-8090.

Our experienced staff will guide you through the process of setting up your first appointment and choosing the right provider for you.  Upon your call, our coordinators will ask you for the following information:

• Reason for the appointment
• Demographic information and contact phone numbers
• Insurance information (please have your insurance card available)
• Custody or guardianship conditions, if applicable

What You Can Expect

Once you have scheduled your first appointment, here’s what you’ll do next:

• For Adults: Please fill out the Adult Patient Questionnaire and bring with you to your first appointment.
• For Children & Adolescents: Please fill out the Child & Adolescent Patient Questionnaire and bring to the first appointment.
• Please arrive at the office 10 minutes early to complete your registration
• Please be prepared to present your identification and insurance cards
• For children and adolescents, please be prepared to provide a copy of any divorce decrees or custody arrangements

Your physician or therapist will meet with you to review your completed Patient Questionnaire and talk with you about the reason for your visit.  The first appointment lasts approximately 45 minutes to 1 hour.

If you are seeing a physician they may prescribe medications and will discuss the benefits and potential side effects with you.  The doctor will also let you know when they would like to see you again.

We recommend that you always schedule your next appointment while still in the office so that we may provide you the best options to meet your schedule.
