  1. How is MyAlexianHealth secure? 
  2. We take great care to ensure your health information is kept private and secure. MyAlexianHealth is hosted on a secure server and accessed via an encrypted, password protected login.

  3. Who is eligible for MyAlexianHealth?
    • You must be 18 years or older or an emancipated minor
    • Possess the legal right and ability to enter into the MyAlexianHealth Patient Portal User Agreement
    • Possess an email account
    • Have had an encounter/visit at Alexian Brothers Medical Center, St. Alexius Medical Center, or Alexian Rehabilitation Hospital

  4. How do I sign up? 
  5. You may enroll in MyAlexianHealth during registration for an inpatient stay or by visiting the Health Information Management Department located at Alexian Brothers Medical Center or Saint Alexius Medical Center. Enrollment must be processed in person and is contingent upon verification of your identity and completion of all appropriate paperwork. Once your enrollment is complete, you will be issued a temporary username and password that will be sent electronically to your email address. This will enable you to login and create your own username and password.

  6. What if I do not receive an email with my temporary username and password?
  7. Check your spam or junk mail folders in your email account. If you still cannot find the invitation, email MyAlexianHealthSupport@ascensionhealth.org, by providing a name and phone number for return service.

  8. How far back will I see my information? 
  9. You will be able to view information as far back as March 2012.

  10. Will I see any behavioral health Information on MyAlexianHealth?
  11. Your medications, diagnoses, and chronic health conditions will be included in your record on MyAlexianHealth. If you have a behavioral health diagnosis or condition documented while at Alexian Brothers Medical Center, Alexian Rehabilitation Hospital, or St. Alexius Medical Center, this information will also be accessible. MyAlexianHealth will not include health information regarding specific notes from group or individual therapy or documentation from a visit to Alexian Brothers Behavioral Health Hospital.

  12. Can my spouse and I share one MyAlexianHealth account?
  13. Due to the sensitive nature of health information, each adult must request and establish his/her own MyAlexianHealth account.

  14. Can I designate my spouse or a family member to have access to MyAlexianHealth record?
  15. Yes, an adult or emancipated minor can grant access to another adult. This is called Proxy Access. You will need to complete and sign a proxy authorization form to release your health information to your designated proxy. Your designated proxy must also be eligible to receive a MyAlexianHealth account.

  16. Can I revoke access to the adult proxy?
  17. Yes, you may revoke Proxy Access at any time by providing a written request to the Health Information Management Department.

  18. Can I access my child’s health information?
  19. For children under the age of 12, a parent or permanent legal guardian may be authorized to view their child’s health information by completing a child proxy access form and submitting all requested documentation. To ensure compliance with applicable privacy laws, proxy access is not available for children from the age of 12-17. This age range limitation in no way limits any legal right you may have to access your child’s health record by other means.
